General Medical Care - Paramus, NJPatient Information About Immigration Physicals

  • Immigration physicals are done on weekdays by appointment preferably early afternoon.
  • The cost is $250 SELF PAY.  Any procedures such as EKG, X-Rays, Tests and Vaccines are extra.  If you need blood work, you will receive a separate bill from the lab.
  • You MUST come to the office with the following documentation before an appointment is made. (All documentation MUST be translated into English and notarized. Once the documentation is reviewed by the physician, someone from our office will call you to set up the appointment.  
  1. A government-issued photo ID (valid unexpired passport, driver’s license or birth certificate if under age 14)
  2. A marriage certificate is required if identification or documentation does not show married name. 
  3. I-693 medical examination form from USCIS (found on website www.uscis.gov/portal/site/uscis).  You must fill out Part 1 only but should not sign. Patient needs to sign forms in the presence of the physician.     
  4. All past medical records (including hospitalizations and institutionalizations, illnesses, disabilities, and treatments)
  5. Immunization records
  • You will not receive the forms back on the same day. All patients are required to have a Tuberculin Skin Test which requires a follow up visit 48 – 72 hours later.   If the Tuberculin Skin Test is positive, a chest x-ray is required.  This can be performed in our office for an additional charge.  The patient that is applying must be the person to return to sign the forms in the presence of the physician.
  • The immigration physical is a complete medical examination with particular attention to communicable diseases.  An external examination of both male and female genitalia is required for anyone 15 years of age and older.  Blood work and urine testing is also required.   
  • The completed I-693 medical examination form must be signed by physician, must contain the official stamp of the office and given to patient in a sealed envelope.
